Job summary

Andwell Health Partners is seeking a Clinical Physical Therapist Case Manager to deliver skilled PT services in clients' homes within the Augusta area. You will assess, plan, and implement care, coordinating with families, physicians, and pay sources to promote independence and well-being.

Eligible candidates hold a Maine PT license, CPR certification, and prefer 1 year of home health experience. A reliable vehicle and insurance are required; flexible day shifts and supportive team environment

Qualifications

Licensed physical therapist with Maine license or eligibility.
Experience in home health or community settings preferred.
CPR certification required.

Responsibilities

Provide physical therapy services to clients according to standards of practice outlined by the American Physical Therapy Association.
Demonstrate knowledge and case management skills to coordinate client home care needs in collaboration with client, family, physician, other providers and pay sources towards outcomes promoting client independence and well-being.
Develop, implement, coordinate, and revise a plan of care that incorporates goals consistent with the client’s age, condition, and needs.
Implement client care plan activities based on treatment goals towards independent living for the client/family.
