Job summary

Andwell Health Partners is seeking a Physical Therapist Case Manager to deliver in-home physical therapy services within Scarborough and surrounding areas. You will assess, plan, coordinate, and implement therapy with a focus on client independence and well-being.

The role emphasizes autonomy, flexible scheduling, and a supportive team with centralized HR and education resources. Maine license and CPR are required, with 1 year PT/home health experience preferred.

Qualifications

• PT degree (Bachelor/Master/Doctorate) from an accredited institution.
• Current PT license in the State of Maine.
• Current CPR certification.
• 1 year PT experience preferred; home health experience preferred.

Responsibilities

• Provide physical therapy services per APTA standards, including screening, evaluation, direct service, consultation.
• Demonstrate case management to coordinate home care with client, family, physician and pay sources.
• Develop and revise a plan of care aligned with client age, condition and needs.
• Implement care activities toward independent living.

What You’ll Do:

• Provide physical therapy services to clients according to standards of practice outlined by the American Physical Therapy Association. Services may include screening, evaluation, direct service, consultation and/or instruction.
• Demonstrate knowledge and case management skills to coordinate client home care needs in collaboration with client, family, physician, other providers and pay sources towards outcomes promoting client independence and well-being.
• Develop, implement, coordinate, and revise a plan of care that incorporates goals consistent with the client’s age, condition, and needs.
• Implement client care plan activities based on treatment goals towards independent living for the client/family.

What You’ll Bring:

• Bachelors, Masters and/or Doctorate Degree in Physical Therapy from an accredited institution.
• Current PT license in the State of Maine.
• Current CPR card required.
• 1-year Physical Therapy experience preferred. Home health experience preferred.
• A reliable car with insurance.
