Job summary

Andwell Health Partners in Norway, ME is seeking a Physical Therapist for full-time hours. The role provides PT services in clients homes and community settings, coordinating care with families, physicians and pay sources to promote client independence and well-being.

Requires a PT degree from an accredited program, Maine license and CPR. A reliable car with insurance is expected; home health experience is preferred. Salary is paid hourly and includes comprehensive health benefits.

Qualifications

• Bachelor, Master and/or Doctorate Degree in Physical Therapy from an accredited institution.
• Current Maine PT license.
• Current CPR card required.
• 1 year PT experience preferred; home health experience preferred.
• Reliable car with insurance.

Responsibilities

• Provide physical therapy services to clients according to standards of practice outlined by the American Physical Therapy Association.
• Demonstrate knowledge and case management to coordinate home care needs with client, family, physician and pay sources.
• Develop, implement, coordinate, and revise a plan of care aligned with client’s age, condition and needs.
• Implement care plan activities based on treatment goals towards independent living for client/family.
