Job summary

NorthLakes Community Clinic is seeking a Care Coordinator to support patients establishing care within the psychiatry program. You will gather intake information, coordinate internal referrals, and connect participants to integrated, patient-centered services.

Role requires a bachelor’s in a social science field and at least two years in behavioral health or social services; a MSW is preferred. Position involves collaboration with the Psychiatry Services Manager and attention to confidentiality.

Qualifications

• Bachelor degree in social science field required with at least 2 years of experience in behavioral health or social services.
• Equivalent combination of education and experience may be considered in lieu of education.

Responsibilities

• Completes screenings and reviews intake materials for recovery program intakes.
• Schedules appointments for psychiatry services and coordinates with clinical staff.
• Assist Psychiatry Services Manager with program access and admission priorities.
• Participate in briefings and program meetings.
• Connect people to whole-person care including social determinants of health.
• Promote timely access to care to reduce ER visits and hospital readmissions.
• Educate participants about relevant recovery resources.
• Maintain HIPAA confidentiality and accurate documentation in EHR.
