Job summary

Sycamore Springs Behavioral Health is seeking a Care Coordination role to assess needs of walk-in and phone callers and refer them to the appropriate care. The coordinator schedules initial psychiatric assessments and supports admission processes, working with the Care Coordination Manager and team to ensure smooth patient transitions.

Required experience in a psychiatric health care setting with strong clinical assessment skills.

Qualifications

• Education: High school degree or equivalent preferred.
• Experience: psychiatric health care facility with direct experience preferred.
• CPR certification and CPI within 30 days.

Responsibilities

• Coordinate referrals and triage patients to appropriate services.
• Schedule initial psychiatric assessments for treatment seekers.
• Document calls, options and appointments for callers seeking treatment.
• Assist in the admission process and support the Care Coordination team.
• Provide clerical support to the Care Coordination Manager.
