Job summary

NorthLakes Community Clinic is seeking a Care Coordinator to support patients establishing care within the psychiatry program. You will gather intake information, coordinate internal referrals, and connect participants to integrated, patient-centered services.

Role requires a bachelor’s in a social science field and at least two years in behavioral health or social services; a MSW is preferred. Position involves collaboration with the Psychiatry Services Manager and attention to confidentiality.

Qualifications

  • Bachelor degree in social science field required with at least 2 years of experience in behavioral health or social services.
  • Equivalent combination of education and experience may be considered in lieu of education.

Responsibilities

  • Completes screenings and reviews intake materials for recovery program intakes.
  • Schedules appointments for psychiatry services and coordinates with clinical staff.
  • Assist Psychiatry Services Manager with program access and admission priorities.
  • Participate in briefings and program meetings.
  • Connect people to whole-person care including social determinants of health.
  • Promote timely access to care to reduce ER visits and hospital readmissions.
  • Educate participants about relevant recovery resources.
  • Maintain HIPAA confidentiality and accurate documentation in EHR.

The Care Coordinator gathers information for individuals establishing care with the psychiatry program, using standard protocols and assessment tools. The Care Coordinator is responsible for supporting and coordinating the needs of program participants, including internal referrals and connection to external resources. The Coordinator engages individuals in their identified goals and connects them to integrated, patient-centered services.

Salary begins at $24.75 + per hour depending on experience.

Essential Job Functions

  • Completes screenings and performs review of intake materials requiring the use of independent judgment to complete recovery program intakes.
  • Schedule appointments fo r patients participating in P sychiatry services and ensure timely communication with clini cal staff regarding appointments.
  • Assist Psychiatry Services Manager with coordinating program access and admission priorities.
  • Participate in briefings, and other program meetings.
  • Work collaboratively with team members to connect people to whole-person care, including clinical needs and social determinants of health.
  • Completes routine screening, brief intervention and referral to treatment to promote service integration.
  • Promote timely access to appropriate care to reduce emergency room utilization and hospital readmissions.
  • Increase utili zation of preventative care.
  • Connect people to resources and supports to engage in identified goals and treatment plans.
  • Mutually establish acceptable boundaries with participants and care team members.
  • Assist Psychiatry Services Manager by serving as an informational resource for participants, care team, family/caregiver(s), and community resources.
  • Facilitate access to app ropriate behavioral health , medical and specialty providers.
  • Increase continuity of care by managing relationships with tertiary care providers, transitions-in-care, and referrals.
  • Educate participants and family/caregiver(s) about relevant recovery resou rces.
  • Facilitate and attend meetings between participant , family/caregiver(s), care team, and community resources, as needed.
  • Assist with population health management initiatives.
  • Participate in Care Coordinator training courses/webinars and meetings.
  • Ass ist Integration team with quality improvement efforts by utilizing dashboards, tracking tools and providing quality reporting data as needed.
  • Supports accurate and timely documentation and audit efforts, using Electronic Health record system for documentation.
  • Understand and demonstrate knowledge of HIPAA and maintain strict confidentiality regarding protected health information.
  • Understands and upholds the organization's stated Mission, Vision, Values and Goals.
  • Prioritize concurrent tasks and team requests to meet tight deadlines; flag obstacles and propose solutions in a timely manner.
  • Other duties as assigned.
Qualifications and Education Requirements

Bachelor degree in social science field (Social Work, Psychology, Sociology, etc.) required ( Mas ters in Social Work preferred) with at least 2 years of experience working in behavioral health or social services .

Equivalent combination of education a nd experience may be considered in lieu of education .
