Elara Caring is hiring a Psychiatric Registered Nurse Home Health to serve Amherst and surrounding areas. The role includes conducting comprehensive patient assessments, managing care plans, and delivering behavioral health guidance during home visits. A $5,000 sign-on bonus is offered.

Required are an active RN license, nursing degree, and 1+ year of clinical nursing experience. A valid driver's license or reliable transportation is essential to travel between patient homes.

Psychiatric Registered Nurse Home Health

$5,000 SIGN ON BONUS

Territories: Amherst, North Hampton, Springfield, Chicopee, Westfield, Holyoke

As a Registered Nurse Behavioral Health , you’ll contribute to our success in the following ways:

• Assess patients’ physical, emotional, social, and environmental needs.
• Manage a caseload and develop, coordinate, and update individualized care plans.
• Complete initial assessments and provide ongoing evaluations during patient visits.
• Monitor patient progress and communicate changes with physicians, supervisors, and the care team.
• Update care plans based on patients’ changing needs.
• Educate patients, families, and caregivers about the plan of care and behavioral health support.
• Teach behavioral health care and symptom management to staff in partner care settings.
• Supervise and support Behavioral Health Aides and LPNs while ensuring quality care.
• Report safety concerns or other issues to supervisors as needed.
• Provide compassionate end-of-life support and complete bereavement assessments.

What is Required?

• Current State license as a Registered Nurse.
• Graduate of an accredited Certificate, Diploma, Associates or Baccalaureate School of Nursing.
• 1+ year of experience as a nurse in a clinical care setting.
• Home Health experience is preferred, but not required.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment.
• Must possess a valid driver’s license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.

The base salary for this position is $85,000 to $90,000 annually based on the company’s good faith estimate at the time of posting. Actual pay will be determined based on factors such as education, experience, skills, and internal equity.
