Job summary

Elara Caring in Amherst, MA is seeking a Behavioral Health Registered Nurse to deliver compassionate home-based care. The RN BH role focuses on comprehensive assessments, care planning, and coordination with the care team to ensure the Right Care at the Right Time.

You will assess, plan, and monitor patient progress, educate families, and support BH staff with best practices. Requires an active RN license and 1+ year clinical nursing experience; home health is preferred.

Qualifications

• Current State RN license.
• Graduate of an accredited nursing program (Certificate, Diploma, ADN/BSN).
• 1+ year nursing experience in a clinical setting.
• Home Health experience preferred.
• Ability to lift up to 50 pounds and travel between patient homes.

Responsibilities

• Develop, coordinate, and update individualized care plans.
• Complete initial assessments and ongoing evaluations during patient visits.

As a Registered Nurse Behavioral Health, you’ll contribute to our success in the following ways:

• Assess patients’ physical, emotional, social, and environmental needs.
• Manage a caseload and develop, coordinate, and update individualized care plans.
• Complete initial assessments and provide ongoing evaluations during patient visits.
• Monitor patient progress and communicate changes with physicians, supervisors, and the care team.
• Update care plans based on patients’ changing needs.
• Educate patients, families, and caregivers about the plan of care and behavioral health support.
• Teach behavioral health care and symptom management to staff in partner care settings.
• Supervise and support Behavioral Health Aides and LPNs while ensuring quality care.
• Report safety concerns or other issues to supervisors as needed.
• Provide compassionate end-of-life support and complete bereavement assessments.

What is Required?

• Current State license as a Registered Nurse.
• Graduate of an accredited Certificate, Diploma, Associates or Baccalaureate School of Nursing.
• 1+ year of experience as a nurse in a clinical care setting.
• Home Health experience is preferred, but not required.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
• Must possess a valid driver’s license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.

The base salary for this position is $85,000 to $90,000 annually based on the company’s good faith estimate at the time of posting. Actual pay will be determined based on factors such as education, experience, skills, and internal equity.
