Elara Caring is seeking a Registered Nurse Behavioral Health in Massachusetts to assess patients’ needs, build care plans, and provide ongoing evaluations across multiple home settings. A $5,000 sign-on bonus is offered. Travel between Amherst, Chicopee, North Hampton, Springfield, Holyoke, and Westfield is required.

The ideal candidate holds an RN license in the state, with 1+ year of clinical nursing experience. A valid driver’s license or reliable transportation is required.

• Current State license as a Registered Nurse.
• 1+ year of nursing experience in a clinical care setting.
• Home Health experience preferred but not required.
• Must be able to lift up to 50 pounds and have a valid driver's license or reliable transportation to travel between patient homes.

Responsabilidades

• Assess patients' physical, emotional, social, and environmental needs.
• Manage a caseload and develop, coordinate, and update individualized care plans.
• Complete initial assessments and provide ongoing evaluations during patient visits.
• Monitor patient progress and communicate changes with physicians, supervisors, and the care team.
• Update care plans based on patients' changing needs.
• Educate patients, families, and caregivers about the plan of care and behavioral health support.
• Teach behavioral health care and symptom management to staff in partner care settings.
• Supervise and support Behavioral Health Aides and LPNs while ensuring quality care.
• Report safety concerns or other issues to supervisors as needed.
• Provide compassionate end-of-life support and bereavement assessments.
