Job summary

Elara Caring is hiring a Psychiatric Registered Nurse (Home Health) to cover Amherst, Holyoke, Springfield, and nearby areas. A $5,000 sign-on bonus is offered.

You will assess patients, manage care plans, and educate families on behavioral health support while coordinating with physicians and the care team. The role requires an active RN license, a nursing degree, and at least one year of clinical nursing experience.

Qualifications

• Current State license as a Registered Nurse.
• Graduate of an accredited Nursing program (Certificate, Diploma, Associate or Baccalaureate).
• 1+ year of nursing experience in a clinical care setting; Home Health preferred but not required.

Responsibilities

• Assess patients' physical, emotional, social, and environmental needs.
• Manage a caseload and develop, coordinate, and update individualized care plans.
• Complete initial assessments and ongoing evaluations during patient visits.
• Monitor patient progress and communicate changes with physicians, supervisors, and the care team.
• Update care plans based on changing patient needs.
• Educate patients, families, and caregivers about the plan of care and behavioral health support.
• Teach behavioral health care and symptom management to staff in partner care settings.
• Supervise BH Aides and LPNs while ensuring quality care.
• Report safety concerns to supervisors as needed.
• Provide compassionate end-of-life support and bereavement assessments.
