Job summary

Illumination Foundation is seeking a Care Coordinator to assess client strengths and needs in the Permanent Supportive Housing program. The role provides trauma-informed care, linkage to medical, mental health, and social services, and maintains accurate records in HMIS and KIPU.

The position requires maintaining confidentiality, supporting client engagement, and coordinating transportation and move-in supports to help clients achieve self-sufficiency.

Qualifications

• Bachelor's degree in Social Services, Healthcare, or a related field or two years of equivalent training and relevant experience.
• Basic computer proficiency including email, documentation, and spreadsheet tools.

Responsibilities

• Maintain a caseload of up to 35+ individuals with case management support.
• Collaborate with care team to ensure integrated, client-centered care.
• Connect clients to essential social services and supports (transportation, food, behavioral health).
• Coordinate discharge planning with hospital staff for smooth transitions of care.
• Develop service plans with measurable goals and regular reassessments.

Education

Bachelor's degree in Social Services, Healthcare, or related field
