Job summary

Illumination Health + Home in Fullerton, CA, is seeking a Care Coordinator to support clients in the Permanent Supportive Housing program. You will assess needs, link to services, and develop individualized housing plans while maintaining confidentiality and trauma‑informed care.

This role emphasizes client engagement and coordination with medical and behavioral health resources. You will document progress in HMIS and KIPU EMR, conduct home visits, and advocate for housing stability, health care

Qualifications

• Bachelor’s degree in Social Services, Healthcare, or related field is required or two years of equivalent experience.
• Experience with HMIS and EMR systems (KIPU) is preferred.
• Ability to manage a caseload and provide trauma‑informed, client‑centered care.
• Proficiency with documentation, data entry, and confidentiality under HIPAA.

Responsibilities

• Maintain a caseload of up to 35+ individuals with consistent case management support.
• Collaborate with Care Team and healthcare providers for integrated, client‑centered care.
• Connect clients with essential services including housing, health care, and financial assistance.
• Coordinate discharge planning with hospital staff for smooth transitions of care.
• Develop and reassess service plans with measurable goals and progress reviews.
• Document client interactions accurately in HMIS and KIPU EMR.
