Job summary

Illumination Health + Home is seeking a Care Coordinator in California to serve as the primary contact for intermittently housed clients. The role focuses on coordinating medical care, behavioral health services, and social supports with a holistic, non-clinical approach.

You will manage up to 30–35 clients, document interactions in EMR/EHR, and collaborate with care teams to remove barriers to care while promoting preventive services.

Qualifications

• Bachelor’s degree in social services, healthcare, or related field or equivalent combination of training and experience.
• Experience in homeless services, case management, and mental health support is preferred.
• Valid California driver’s license required to travel to multiple locations with clients.

Responsibilities

• Provide comprehensive case management by assessing client needs, developing individualized treatment plans, monitoring progress, and following up on goals.
• Document care plans and progress in EMR/EHR systems with accuracy and confidentiality.
• Collaborate with other departments through meetings to evaluate program effectiveness and improve treatment plans.
• Connect clients to resources supporting psychosocial and daily needs, including healthcare, nutrition, and housing referrals.
• Accompany clients to medical appointments and coordinate transportation as needed.
• Maintain a caseload of up to 30–35 clients and prepare for supervision sessions.
• Submit daily EOS reports and monthly tally sheets to document performance metrics.
