Job summary

Illumination Health + Home is seeking a Care Coordinator to assess client needs in the Permanent Supportive Housing program and link them to housing and service resources. The role emphasizes trauma-informed care, active listening, and helping clients achieve self-sufficiency.

Responsibilities include maintaining records, coordinating transportation and services, and supporting clients through move-ins and ongoing engagement.

Qualifications

• Bachelor’s degree in Social Services, Healthcare, or related field.
• Minimum of two years of equivalent training and relevant experience.
• Valid California driver’s license with clean driving record.
• Must be eligible for company vehicle insurance.

Responsibilities

• Maintain a caseload of up to 35+ individuals with case management support.
• Collaborate with Care Team members to ensure integrated, client-centered care.
• Connect clients to essential social services and supports.
• Coordinate discharge planning with hospital staff for smooth transitions.
• Develop service plans and conduct regular reassessments.
• Document client progress in HMIS and EMR with accuracy.
