Job summary

Illumination Health + Home is seeking a Care Coordinator for the Permanent Supportive Housing program. You will assess client strengths, connect to housing and services, and document progress in HMIS and KIPU EMR, while upholding confidentiality.

Responsibilities include managing up to 35 clients, coordinating with care teams, supporting move-ins, and advocating for housing, healthcare, and social supports. Bilingual Spanish speakers encouraged; CA driver license required.

Qualifications

• Bachelor’s degree (or 2 years equivalent experience) in a related field.
• Basic computer proficiency, including Excel.
• Valid California driver's license and clean driving record.
• Must be eligible for company vehicle insurance coverage.
• Preferred bilingual Spanish ability for client communication.
• Experience in non-profit housing or housing programs is a plus.
• Knowledge of HMIS and data entry in EMR systems.

Responsibilities

• Maintain a caseload of up to 35+ individuals with consistent case management.
• Collaborate with care team to coordinate integrated client-centered care.
• Connect clients to transportation, food assistance, and behavioral health supports.
• Coordinate discharge planning with hospital staff for smooth transitions.
• Develop service plans and perform regular reassessments to ensure progress.
• Conduct home visits, wellness checks, and engagement activities.
• Assist with job search, benefits applications, and skills programs.
• Advocate for housing, healthcare, and supportive services access.
• Ensure housing habitability standards and privacy compliance.
