Job summary

Illumination Health + Home is seeking a Care Coordinator to assess client needs in the Permanent Supportive Housing program and develop individualized housing and service plans. The role emphasizes trauma-informed care, active listening, and documenting progress in HMIS/KIPU with strict confidentiality.

The position supports connecting clients to medical, mental health, substance use treatment, and financial resources, while promoting independence and community integration.

Qualifications

• Bachelor’s degree in Social Services, Healthcare, or related field.
• Minimum of two years of equivalent training and relevant experience.
• Proficiency with basic computer skills including email, documentation, and data summarization.

Responsibilities

• Maintain a caseload of up to 35+ individuals with consistent, individualized case management.
• Collaborate with Care Team members to ensure integrated, client-centered care.
• Connect clients to essential social services and supports such as transportation and health resources.
• Coordinate discharge planning with hospital staff to ensure smooth transitions.
• Develop and reassess comprehensive service plans to meet evolving needs.
• Maintain accurate documentation in HMIS and KIPU EMR, ensuring confidentiality.
