Job summary

PathForward in Arlington, VA seeks a Permanent Supportive Housing Case Manager for a full-time role supporting PSH program participants to obtain and maintain stable housing.

The role reports to the VP, Housing and requires on-site work at Arlington Mill Residences with a team providing on-site support. Strong collaboration, documentation, and coordination with service providers are essential.

Qualifications

• Must be a self-starter.
• Excellent written and oral communication skills.
• Excellent interpersonal and organizational skills.
• Excellent computer skills and experience with Outlook and Microsoft Office.
• Demonstrated ability to work well with individuals in crisis.
• Empathy, patience and persistence are required.
• Must have a valid driver’s license and clean driving record.

Responsibilities

• Engage eligible program participants when there is a vacancy.
• Assist participants in filling intake referral applications and collecting documentation.
• Coordinate with Housing Locator to identify available affordable units.
• Assist with leasing applications and moving in to units after approval.
• Schedule weekly home visits to support successful tenancy.
• Maintain weekly case notes and participant files with up-to-date service plans.
