The role emphasizes outreach-based services with a focus on harm reduction, connection to community supports, and helping participants stabilize in housing. You will assess needs, coordinate with landlords and agencies, maintain HMIS documentation, and produce monthly case management reports.

Qualifications

• Bachelor’s degree in a related field OR 2 years of experience in social services.
• Ability to interact with other agencies and advocate for participants.
• Knowledge of chemical dependency and mental health issues and resources.
• Experience working with property/building managers and maintaining relationships.
• Mediation between agencies, participants, and property/building managers.
• Up-to-date knowledge of community resources for homelessness.
• Knowledge of domestic violence and sexual assault resources.
• Knowledge of services for homelessness or housing insecurity and veterans.
• Ability to work as part of a team and maintain participant files.
• Computer literacy including word processing, database, spreadsheet, email, and internet.
• Ability to prepare statistical and narrative reports.
• Bilingual English/Spanish preferred. Veterans may receive priority.
• Valid driver's license and reliable transportation.

Responsibilities

• Provide outreach-based support to people in Permanent Supportive Housing for housing stability.
• Assess service needs including medical, mental health, substance use, income, education, and basic needs.
• Coordinate connections to community providers and landlords to advocate for participants.
• Meet with participants to deliver and coordinate services.
• Maintain participant files and HMIS documentation.
• Attend staff, case management, and partner meetings as assigned.
• Develop and maintain relationships with community resources to improve access.
• Adhere to budgetary guidelines and provide monthly case management reports.
