Position Description:

The Permanent Supportive Housing (PSH) Case Manager, as a member of Catholic Community Services (CCS) Family Housing Network, provides intensive, client-centered case management services to individuals and families experiencing chronic homelessness and housing instability. This role supports residents in maintaining stable housing while addressing barriers such as behavioral health needs, substance use, medical conditions, and limited income. The Case Manager uses a housing-first, trauma-informed, and strengths-based approach to promote self-sufficiency and overall well-being.

Responsibilities

Key Responsibilities

  • Case Management & Service Delivery
  • Using motivational interviewing, conduct comprehensive assessments and develop client-driven Housing Stability Plans with measurable goals.
  • Provide ongoing, individualized case management to residents in permanent supportive housing programs to monitor progress towards housing stability, adjust goals and actions plans as needed.
  • Assist clients in accessing community resources, including healthcare, mental health services, substance use treatment, benefits, and employment services.
  • Maintain regular contact with clients through home visits, office meetings, and field outreach per program guidelines and contract deliverable expectations.
  • Housing Stability Support
  • Consistently post housing availability to PSH clients through the Coordinated Entry System.
  • Support residents in maintaining lease compliance and resolving landlord/tenant issues.
  • Provide education on tenant rights and responsibilities.
  • Intervene early to prevent evictions, including crisis intervention and conflict resolution.
  • Coordinate with property management to ensure safe and stable housing environments by conducting unit inspections per established guidelines, communicating concerns to the property manager.
  • Documentation & Compliance
  • Maintain accurate, timely, and confidential client records in compliance with organizational, program and contract funder requirements.
  • Complete required documentation such as intakes, assessments, service plans, regularly occurring meeting notes, referrals, progress notes and other reports in accordance with organizational, program and contract funder requirements.
  • Utilize Homeless Management Information System (HMIS) or similar databases.
  • Collaboration & Coordination
  • Work collaboratively with internal teams (property management, outreach staff, clinical providers) and external partners.
  • Participate in case conferences, multidisciplinary team meetings, and community coalitions.
  • Advocate for clients within systems of care, including healthcare, legal, and social services.
  • Crisis Intervention
  • Using trauma informed practice, respond to client crises, including mental health emergencies, substance use relapse, and other threats to housing stability and document accordingly.
  • De-escalate situations using trauma-informed approaches and connect clients to appropriate emergency services when needed.
Qualifications

Minimum Qualifications

  • Minimum 1-year experience as Case Manager with:
    • Experience providing services to high needs, low-income/no-income families who have experienced housing instability/homelessness, behavioral health challenges or similar while maintaining healthy boundaries.
    • Knowledge of community resources and social service systems.
    • Proficiency in basic computer applications and database management systems.
  • And Minimum of either of the following experience and education
    • A total of 5 years’ experience working in Human Services or Social Services performing duties aligned with the major duties and responsibilities of the role.

  • Associates Degree in Social Services, Psychology, Human Services, or a related field and 1-year experience in Human Services or Social Services performing duties aligned with the major duties and responsibilities of the role.
  • Upon contingent offer of employment, the applicant must successfully pass required background checks and provide three acceptable references.
  • Must have reliable transportation and provide valid driver's license, automobile insurance and current auto registration.
  • Must have an acceptable driving record per CCS' driving policy, (required to provide current driving abstract).
  • Proof of negative TB test upon hire or proof of test within past 12 months.

Preferred Qualifications

  • Bachelor’s degree in Social Services, Human Services, Psychology or related field.
  • Experience in Housing First and/or Permanent Supportive Housing models.
  • Experience with trauma-informed care, harm reduction, and motivational interviewing.
  • Experience with housing services, landlord/ tenant law and other housing issues.
  • Knowledge of Pierce County social services and other community resources.
  • Bilingual or multilingual skills.
  • Familiarity with HMIS or similar data systems
