Catholic Charities of the Diocese of Fall River is seeking a Permanent Supportive Housing (PSH) Case Manager for a temporary, full-time role in Fall River, MA. The position requires strong organizational and communication skills, driving to multiple sites, and proficiency with the ASSIST system and MS Office tools.

Qualifications

• BA/BS in Human Services preferred or combination of education and experience.
• Strong organizational, assessment, and communication skills.
• Proficiency with ASSIST, Outlook, Word, and Excel.
• Reliable, insured vehicle and valid driver’s license for travel to scattered sites.
• Bi-lingual skills are a helpful plus.
• Commitment to ending homelessness and client confidentiality.

Responsibilities

• Complete initial client needs assessments and develop client-driven case plans with short- and long-term goals.
• Connect clients with employment, public assistance, education, mental health, and other resources.
• Meet with clients weekly to evaluate progress and address barriers.
• Conduct unit inspections, coordinate inspections with maintenance, and manage screening steps.
• Maintain accurate documentation and manage case files using ASSIST, Outlook, Word, and Excel.
• Participate in on-call rotation to respond to emergencies and client needs.
