Job summary

Catholic Charities of the Diocese of Fall River is hiring a Permanent Supportive Housing (PSH) Case Manager in Fall River, MA. This full-time position (temporary) focuses on assessing client needs, developing case plans, connecting clients to resources, and supporting housing stability in a community setting.

The role requires strong organization, communication, and case-management skills; a BA/BS in Human Services is preferred.

Qualifications

BA/BS in Human Services preferred or equivalent combination of education and experience in human services, housing, and homelessness services.
Excellent organizational, assessment, and communication skills.
Computer proficiency with the ASSIST computer program, Outlook, Word, and Excel.
A reliable, insured vehicle and valid driver’s license required for travel to scattered sites.
Bi-lingual skills are a helpful plus.
Commitment to ending homelessness, client confidentiality, and safety/EOHLC requirements.

Responsibilities

Complete initial client needs assessments and develop client-driven case plans with short- and long-term goals.
Connect clients with community resources to meet employment, education, mental health, and housing needs.
Meet with clients weekly to evaluate progress and strategize.
Coordinate unit inspections and screening steps (CORI forms, credit checks) for new clients.
Maintain accurate documentation and manage case files using ASSIST, Outlook, Word, Excel.
Participate in on-call rotation and respond to emergencies and client needs.
