Primary Health Care is seeking a Program Support Specialist for its Homeless Support Services in Des Moines, IA. You will provide administrative support across grant-funded programs, assist with basic needs services, data entry, and quality improvement projects to meet program goals and standards.

Responsibilities include greeting clients, completing registrations, answering calls, and coordinating with PHC teams and external agencies to deliver positive service experiences.

Qualifications

• High school diploma or equivalent required.
• Strong customer service and administrative skills.
• Experience in healthcare or human services preferred.
• Basic computer proficiency (email, word processing, spreadsheets).
• Fluency in English; Spanish is a plus.

Responsibilities

• Welcomes clients and department visitors and provides assistance, referring to resources as needed.
• Completes accurate registration for clients receiving services.
• Answers phone calls and delivers messages per protocol.
• Assists clients with shower, clothing, supplies, and other services.
• Advocates for clients and researches community resources.
• Provides external referrals for PHC services.
• Enters case notes and program data into HMIS per grant guidelines.
• Performs other duties as assigned.
