Job summary

Primary Health Care (PHC) Homeless Support Services seeks a Program Support Specialist to provide administrative support for grant-funded programs, assist with basic need services, data entry, and participate in quality improvement activities that help programs meet goals.

You will greet clients, register them, answer calls, refer resources, and document in HMIS while coordinating with PHC teams and community partners to ensure compassionate, accurate service.

Qualifications

• High school diploma or equivalent.
• 1 year of healthcare or human services experience, or 2 years in customer service.
• Associate degree or experience in human services preferred.

Responsibilities

• Welcomes clients and department visitors; provides prompt, friendly customer service.
• Completes accurate registration for clients receiving services.
• Answers phone calls and delivers messages per protocol.
• Assists clients with shower, clothing, supplies, and other services.
• Advocates for clients and researches community resources as needed.
• Enters case notes and program data into HMIS per grant guidelines.
