Supportive Concepts for Families in Carnegie, PA is seeking a Program Specialist to maintain caseloads for residents, coordinate services, and ensure quality supports. The role involves ISP meetings, documentation, and collaboration with families and coordinators.

A 4-year experience and an associate degree are expected, with a preferred bachelor’s degree and on-site work. The position offers a $56,000 annual salary, a $2,000 sign-on bonus, paid time off, and benefits including DailyPay and

Qualifications

• Associate's Degree (or 60 credit hours) and four years of relevant experience.
• Excellent communication, documentation, and organizational skills.
• Detail-oriented with strong problem-solving abilities.
• Valid driver's license and reliable transportation.
• Proficiency with Microsoft Office and electronic health record systems.
• Ability to work independently and with interdisciplinary teams.
• Bachelor's Degree and two years of related experience preferred.

Responsibilities

• Maintain a caseload for individuals receiving residential services.
• Complete quarterly reports, annual assessments, and other required documentation.
• Attend ISP meetings and develop annual outcomes and supports.
• Serve as liaison between individuals, families, Supports Coordinators, and partners.
• Visit homes to monitor services and ensure quality supports.
• Coordinate evaluations and services including therapy and equipment.
• Monitor records to ensure state regulatory compliance.
• Participate in waiver monitoring, licensing reviews, and QA activities.
• Assist with behavior support planning and crisis responses.
• Track medical appointments and documentation in electronic records.
• Partner with coordinators, families, and team members to promote outcomes.
