Job summary

Supportive Concepts for Families is seeking a dedicated Program Specialist to manage residential caseloads and drive person-centered planning in Carnegie, PA. You will coordinate ISP meetings, outcomes, and services across multiple lines within the Supports coordination framework.

Qualifications

  • Associate's Degree (or 60 credit hours) and four years of experience working directly with individuals with developmental disabilities.
  • Excellent communication, documentation, and organizational skills.
  • Detail-oriented with strong problem-solving abilities.
  • Valid driver's license, reliable transportation, current vehicle registration, and vehicle insurance.
  • Proficiency with Microsoft Office and electronic health record systems.
  • Ability to work independently and collaboratively with interdisciplinary teams.

Responsibilities

  • Maintain a caseload of individuals receiving residential services.
  • Complete quarterly reports, annual assessments, outcomes, SNAP assessments, personal inventories, consumer demographic information, and other required documentation.
  • Attend ISP meetings and develop annual outcomes, procedures, and person-centered supports.
  • Serve as a liaison between individuals, families, Supports Coordinators, Administrative Entities, and community partners.
  • Visit assigned homes regularly to monitor services, assess needs, and ensure quality supports are being provided.
  • Coordinate evaluations and services including physical therapy, occupational therapy, speech services, adaptive equipment, and specialized supports.
  • Monitor individual records and documentation to ensure compliance with state regulations and agency standards.
  • Participate in waiver monitoring activities, licensing reviews, SIS assessments, quality assurance visits, and other regulatory reviews.
  • Assist with behavior support planning, crisis response, and training related to individual support plans.
  • Track medical compliance, appointments, annual assessments, and required documentation within electronic record systems.
  • Partner with Program Coordinators, Assistant Program Coordinators, families, and interdisciplinary team members to promote positive outcomes and quality services.
  • Participate in on-call responsibilities as assigned.

What You'll Do

As a Program Specialist, you will:

  • Participate in waiver monitoring activities, licensing reviews, SIS assessments, quality assurance visits, and other regulatory reviews.

Physical Requirements
