Job summary

Easterseals PORT Health in New Bern, NC is seeking a Service Coordinator for a full-time role. You'll supervise developmental disabilities services for EMPOWER programs, ensuring person-centered supports and compliance with state and agency standards.

Responsibilities include enrollment documentation, consent management, referral processing, and progress reporting to program supervisors and families. A valid driver’s license and relevant experience are required.

Qualifications

• Bachelor's degree from a college or university accredited by USDOE or CHEA.
• Minimum of Two (2) years’ experience working with individuals with Intellectual and / or Developmental Disabilities, and meets qualifications for Qualified Professional.
• Must have a valid, unrestricted, state appropriate driver’s license and personal vehicle insurance.
• Satisfactory criminal background check and acceptable motor vehicle record.
• Knowledge of programs such as Microsoft Word, Microsoft Excel, Zoom and Microsoft Teams.

Responsibilities

• Supervise developmental disabilities services for individuals enrolled in the EMPOWER programs at assigned locations.
• Ensure best practices, supports based on personal outcomes, and compliance with state, federal, and agency standards.
• Complete required enrollment documents for EHR and Provider Direct and forward to Billing Specialist.
• Secure necessary consents and authorizations before supports begin and keep services within authorized limits.
• Develop person-centered plans and short-range goals, consents, and NCSNAP as required.
• Report progress to program supervisor, staff and families quarterly, using available resources to meet participant needs.
