Job summary

Easterseals PORT Health in New Bern is seeking a full-time Service Coordinator to supervise developmental disabilities services for individuals enrolled in EMPOWER programs such as Recreation, Wellness and Transition. You will ensure best practices and supports aligned with personal outcomes and compliant with state, federal, and agency standards.

Requirements include a Bachelor’s degree, at least two years’ experience with Intellectual/Developmental Disabilities, a valid driver’s license,

Qualifications

• Bachelor’s degree from an accredited college or university.
• Minimum 2 years’ experience with Intellectual and/or Developmental Disabilities.
• Valid, unrestricted driver’s license and personal vehicle insurance.
• Satisfactory background check and motor vehicle record.
• Familiarity with Microsoft Word, Excel, Zoom and Teams; EHR experience a plus.

Responsibilities

• Supervise developmental disabilities services for Empower program participants.
• Complete enrollment documents for EHR and Provider Direct; forward to Billing.
• Ensure consents and authorizations are in place before supports begin.
• Develop person-centered plans and short-range goals; manage referrals.
• Report progress to program supervisor and families quarterly.
• Collaborate with MCO and other entities to align supports with needs.
