Job summary

Easterseals PORT Health is seeking a Service Coordinator to supervise developmental disabilities services for individuals in the EMPOWER programs at New Bern locations. You will ensure best practices, outcomes-based supports, and compliance with state, federal, and agency standards.

The role requires coordinating enrollment documentation, authorizations, and referrals, while tracking progress and collaborating with MCOs and other service lines to meet participant needs and preferences.

Qualifications

• Bachelor’s degree from a college or university accredited by USDOE or CHEA.
• Minimum of two (2) years’ experience working with individuals with Intellectual and/or Developmental Disabilities.
• Must have a valid, unrestricted driver’s license.
• Satisfactory criminal background check and acceptable motor vehicle record.
• Knowledge of Microsoft Word, Microsoft Excel, Zoom and Microsoft Teams.

Responsibilities

• Supervise developmental disabilities services for individuals enrolled in the Empower programs at assigned locations.
• Complete enrollment documents for EHR and Provider Direct and forward to Billing Specialist.
• Ensure necessary consents and authorizations are in place before supports begin and services stay within authorized limits.
• Develop person-centered plans and short-range goals, consents, and NCSNAP as required.
• Report progress towards goals to program supervisor, staff and families quarterly using available resources to meet participant needs.
• Collaborate with MCO and other entities to ensure supports align with desires and needs of individuals.
