Job summary

The People Concern is partnering with DHS HFH to provide housing and Intensive Case Management Services (ICMS) to people experiencing homelessness. The program-based housing houses clients in subsidized units with attached services and trauma-informed care.

ICMS teams offer home- and field-based interventions while collaborating with partners to meet funder standards. The Program Manager oversees daily ICMS operations, supports crisis management, coordination of services, and clinical

Qualifications

• MSW, MFT, LPCC or PsyD registered with CA BBS; supervision hours available for this position.
• Licensed preferred, but must be license eligible.
• Minimum of two years’ full-time experience working with the homeless, preferably with mental illness or substance use.
• Able to provide direct supervision and management of assigned staff.
• Detail oriented with excellent time management, organizational, written, verbal, interpersonal, and computer skills.

Responsibilities

• Provide clinical and administrative supervision to Bachelors-level Case Managers and Masters-level Clinical Case Managers.
• Facilitate weekly team meetings and case consultation using trauma-informed care and harm reduction.
• Collaborate with Housing Department teams, DHS supports, and community partners to improve safety and well-being for clients.
• Assist with client crisis management including assessment for psychiatric hospitalization.
• Be on-call 24/7 with support from supervisor and Housing Directors.
• Ensure documentation and data entry for clients is complete and audited.
• Provide budgetary oversight of program administration within the annual budget.
• Hire and manage staffing of ICMS team and ensure policy adherence.
• Assist in screening, assessment, and enrollment of clients into the program.
• Link clients to services and ensure access to housing and benefits.
