Jefferson Health is seeking a Program Manager for Population Health within Employee Benefits– Employee Health. The role focuses on leading MSO client initiatives, collaborating with Population Health MSO, MSO Clients, Jefferson Medical Group, Community Impact and Finance to drive innovation, operationalize programs, and measure impact on preventive care and chronic disease management.

Qualifications

• Bachelor’s Degree required.
• Experience leading MSO programs or population health initiatives preferred.
• Strong data analysis and reporting skills.
• Excellent communication and stakeholder management abilities.

Responsibilities

• Analyzes and monitors employee health benefit and population health data to identify improvement opportunities.
• Develops scorecards and reports to track MSO client population initiatives with actuarial/analytics teams.
• Leads plans to meet/exceed client outcome objectives across Benefits, Population Health, Community Impact, and Finance.
• Acts as lead contact for executive teams regarding MSO initiatives for employee clients.
• Provides insights on employee health benefit regulations, trends, and innovations to stakeholders.
• Directs and manages multiple projects through development, implementation, and evaluation.
• Builds and maintains strong relationships with clients to understand business processes and objectives.
• Leads cross-campus and departmental teams to meet timelines and budget.
