Job summary

Jefferson Health in Philadelphia is seeking a Program Manager for Population Health within Employee Benefits to lead MSO client initiatives and partner with Jefferson Medical Group, Community Impact, and Finance teams.

You will analyze population health data, develop scorecards, and guide cross-functional projects to improve preventive care and chronic disease management for employee health plan members and contracted populations.

Qualifications

• Bachelor’s Degree is required.

Responsibilities

• Analyzes and monitors employee health benefit and population health data to identify program improvements.
• Develops scorecards and reports to measure MSO client population initiatives with actuarial/analytics teams.
• Leads plans to meet/exceed client outcome objectives with Benefits, Population Health, Community Impact and Finance teams.
• Serves as lead contact for executive teams regarding MSO initiatives for employee clients.
• Manages multiple projects through development, implementation and evaluation.
• Builds strong relationships with clients to understand their business processes and objectives.
• Mobilizes cross-campus project teams to meet objectives on time and within budget.
