Qualifications

• Requires nursing degree and active RN license in PA/NJ with licensure upon hire.
• Strong communication skills to facilitate reviews with providers.
• Experience with inpatient acute care and care progression workflows.

Responsibilities

• Oversee care progression across interdisciplinary teams to ensure timely progression and safe discharge planning.
• Identify clinical conditions and interventions suitable for ambulatory follow-up with proper follow-up.
• Apply medical criteria to monitor necessity, reduce unnecessary days, and collaborate with physicians and utilization management.
• Contribute to performance improvement initiatives with care management and clinical teams.
• Monitor metrics like length of stay and readmissions to improve efficiency and outcomes.
