La Clinica de Familia (LCDF) is expanding an Intensive Outpatient Program (IOP) team. This position will monitor and coordinate the activities of the assigned Patient Management Team, in partnership with the Clinical Supervisor, ensuring proper utilization of services and equipment.

The role requires strong communication, organization, and knowledge of health management, with a focus on serving diverse, low-income communities in Southern New Mexico.

Qualifications

Associate degree (min. 2 years college) plus one year in similar position or high school diploma plus two years experience in similar position.
Ability to establish positive working relationships with clients, staff, colleagues, and community members.
Thorough knowledge of community resources, agency services, and funding requirements.

Responsibilities

Coordinate activities of the Patient Management Team under Clinical Supervisor guidance.
Ensure utilization of services and equipment complies with program guidelines.

Job Summary:

With the expansion of LCDF services as a newly designated Certified Behavioral Health Community Center, we are creating an Intensive Outpatient Program (IOP) team. This position will be responsible for monitoring and coordinating the activities of the assigned Patient Management Team, in partnership with assigned Clinical Supervisor, in compliance with established utilization of services and equipment

Core Competencies:

Must be a self-initiating and adaptable with an ability to communicate to a variety of staff members.
Must be able to follow directions well.
Must possess good oral and written communication skills.
Must be highly organized. Must have high attention to detail.
Must have excellent math skills. Must maintain a high level of confidentiality.
Must have basic understanding of IBM computers and Windows, Microsoft Word, Word Perfect, variety of Spread sheet programs; RAMS MIS system a plus.
Knowledge in health management and service delivery.
Knowledge of and experience with the poverty level population and life-styles.

Job Requirements:

Associates degree (min. 2 years college) plus one year experience in similar position or high school diploma (or equivalent) plus two years experience in similar position. Must have minimum one year experience in a supervisory role.
