Job summary

La Clinica de Familia (LCDF) is expanding an Intensive Outpatient Program (IOP) team. This position will monitor and coordinate the activities of the assigned Patient Management Team, in partnership with the Clinical Supervisor, ensuring proper utilization of services and equipment.

The role requires strong communication, organization, and knowledge of health management, with a focus on serving diverse, low-income communities in Southern New Mexico.

Qualifications

• Associate degree (min. 2 years college) plus one year in similar position or high school diploma plus two years experience in similar position.
• Ability to establish positive working relationships with clients, staff, colleagues, and community members.
• Thorough knowledge of community resources, agency services, and funding requirements.

Responsibilities

• Coordinate activities of the Patient Management Team under Clinical Supervisor guidance.
• Ensure utilization of services and equipment complies with program guidelines.
• Collaborate with staff to support program expansion and quality care.
