Job Title: Program Director, Community Supports – Private Duty

Reports to: Senior Director, Community Care Hub

FLSA Status: Full-time, Exempt

Position Summary:

The Program Director, Community Supports – Private Duty provides strategic and operational leadership for Partners in Care Foundation’s Private Duty Services portfolio, including Respite, Personal Care, and Homemaker Services delivered through Medi-Cal Community Supports, managed care contracts, and other funding sources.

Reporting to the Senior Director, Community Care Hub, the Program Director oversees program operations, provider network performance, health plan relationships, quality and compliance, financial performance, and sustainable growth while ensuring high-quality, person-centered service delivery.

Minimum Qualifications:

  • Bachelor’s degree in Public Health, Social Work, Healthcare Administration, Gerontology, Business Administration, Nursing, or a related field.
  • Five (5) years of progressively responsible leadership experience in healthcare, aging services, home-and community-based services (HCBS), long-term services and supports (LTSS), managed care, home care, or related fields.
  • Experience managing complex programs, contracts, provider networks, or service delivery operations.
  • Experience supervising staff and managing multidisciplinary teams.
  • Knowledge of Medi-Cal, CalAIM, Community Supports, LTSS, HCBS, or related programs.
  • Strong operational, financial, analytical, communication, and stakeholder-management skills, including the ability to use data to monitor performance and inform decision-making.

Preferred Qualifications:

  • Master’s degree in a related field.
  • Experience managing Medi-Cal Community Supports, HCBS/LTSS, home care, or similar services.
  • Experience working with managed care plans, Community Care Hubs, and/or community-based provider networks.
  • Experience with payer contracts, financial performance, and service delivery operations.

Responsibilities:

Program Leadership and Operations
  • Provide strategic leadership and day-to-day oversight of Private Duty Services: including Caregiver Respite, Personal Care, and Homemaker Services.
  • Develop and implement operational plans, policies, procedures, workflows, and performance improvement initiatives aligned with contractual and organizational requirements.
  • Monitor service delivery, utilization, productivity, participant satisfaction, and performance metrics; identify and implement opportunities to improve efficiency and outcomes.
  • Lead implementation of service expansions, and operational improvements that support organizational goals.
Provider Network and Service Delivery
  • Oversee relationships with contracted service providers, home care agencies, community-based organizations, and other network partners.
  • Ensure adequate provider capacity, quality, geographic coverage, documentation, and compliance to meet referral demand and contractual requirements.
  • Collaborate with program management on provider recruitment, onboarding, credentialing, training, engagement, and performance management.
  • Develop strategies to improve provider satisfaction, retention, service quality, and fulfillment performance.
Health Plan and Contract Management
  • Serve as the operational lead for health plan relationships related to Private Duty Community Supports services.
  • Monitor contractual performance measures, service-level agreements, turnaround times, fulfillment rates, and member outcomes, and address identified service gaps or operational challenges.
  • Maintain dashboards and reports to support performance monitoring, audit readiness, and health plan performance reviews.
  • Support implementation of new contracts, reimbursement arrangements, and external stakeholder partnerships.
Quality, Compliance, and Community Care Hub Integration
  • Ensure compliance with applicable federal, state, health plan, accreditation, contractual, and organizational requirements.
  • Oversee quality assurance and improvement activities, including participant satisfaction, service outcomes, incident reporting, complaints, corrective actions, documentation, and audit readiness.
  • Collaborate with Community Care Hub leadership and internal teams to integrate Private Duty Services with care coordination, Community Supports, Community Health Worker, and population health initiatives.
  • Develop and strengthen workflows that support seamless referrals, care transitions, coordinated services, and whole-person care.
Financial Management and Strategic Growth
  • Develop and manage program budgets and monitor revenue, expenses, utilization, service costs, and overall financial performance.
  • Analyze referral trends, staffing needs, provider reimbursement structures, and utilization data to support sustainable operations and informed decision-making.
  • Contribute to rate development, contract negotiations, forecasting, and growth planning while maintaining service quality and member satisfaction.
Staff Leadership and Development
  • Supervise assigned program staff and establish clear performance expectations and accountability measures.
  • Provide coaching, mentoring, and professional development to support strong performance, retention, and succession planning.
  • Foster a culture of accountability, collaboration, continuous learning, innovation, and service excellence.

Physical Requirements:

  • Ability to work in a hybrid office and community-based environment.
  • Ability to travel locally and throughout California as needed.
  • Ability to sit, stand, and use a computer for extended periods.
