  • Medical or Clinical professional experience, ideally within nursing or social work or other healthcare service delivery.
  • This position is remote, but candidate must reside in California.
  • Travel up to 20-25%, dependent on California location.

Responsibilities

  • Direct program through objectives and goals for contract compliance; monitor metrics and daily operations.
  • Collaborate with medical director, management teams, and corporate support to improve processes and training.
  • Engage community partners and stakeholders in delivering member outreach.
  • Provide leadership and supervision to staff, ensure accreditation standards compliance.
  • Oversee contract scope, RFP cycles, and related new business opportunities.
  • Oversee budget review and analysis for program financial success.
  • Assess systems and processes to ensure high customer satisfaction.
  • Help design organizational structure for efficient processes and California outreach.

Job Summary

The Program Director provides strategic and operational leadership for a comprehensive Care Coordination program serving members across California. This role is responsible for overseeing program design, day‑to‑day operations, workforce performance, quality outcomes, regulatory compliance, and service delivery across interdisciplinary teams. The Program Director partners with clinical, operational, provider, health plan, and community‑based organizations to ensure members receive timely, equitable, and person‑centered support. Responsibilities include managing care coordination workflows; strengthening transitions of care; addressing social drivers of health; supporting complex and high‑risk populations; monitoring access, engagement, and outcomes.

Responsibilities
  • Effectively directs program through objectives and goals assigned for contract compliance. Monitors metrics and daily operations to ensure success.
  • Collaborates with program leaders, including medical director, management teams, and corporate support teams to improve processes, determine training needs and improvement opportunities.
  • Engages community partners and stakeholders in delivering member outreach
  • Provides leadership and supervision to staff assigned to the program, including quality, Case Management, Care Coordination, provider training/education, consumer education, data collection, analysis and reporting, and technical assistance, to ensure compliance with accreditation standards.
  • Works with management and corporate team for the enhancement of the contract scope, new RFP cycles and related new business.
  • Oversees budget review and analysis to ensure the financial success of the program.
  • Assesses all systems, including contract, policy and procedure, workflow, etc., continually to ensure a high level of customer satisfaction to all stakeholders.
  • Collaborates with management team to create an organizational structure that will ensure effective processes and work designs, including team integration and oversight which includes California field‑based outreach and localized community engagement, within the southern region.
  • Assists in determining professional development opportunities for staff and teams.
  • Read, understand, and adhere to all corporate policies including policies related to HIPAA and its Privacy and Security Rules.

Preferred Qualifications
  • 3+ years in case management/care coordination
  • Previous experience in developing community‑based member engagement strategies
  • Provider Network experience either through provider relations or provider recruitment/enrollment
  • Knowledge of California healthcare delivery systems, including Medi‑Cal and MCO environments
  • Budget and financial skills
  • experience in developing stakeholder relationships.

Experience In Lieu Of Degree

For non‑clinical roles, or when not required by the contract specifically, the Company acknowledges that practical, hands‑on experience can provide skills and competencies equivalent to formal education. As such, in cases where a Bachelor’s degree may be required, the Company will accept a minimum of six (6) years of directly relevant professional experience in lieu of a degree. In instances where the candidate has an Associate’s degree, the Company will accept a minimum of three (3) years of directly relevant professional experience in lieu of the Bachelor’s degree.

The pay range for this position is listed below. “Based on our compensation philosophy, an applicant’s position placement in the pay range will depend on various considerations, such as years of applicable experience and skill level.”
