CP unlimited in New York is seeking a qualified program administrator to oversee residential services while ensuring compliance with regulatory standards. You will develop and monitor individualized program plans, chair and participate in life plan conferences, and maintain thorough documentation for all residents.

The role requires a BA/BS in a related human services field, at least 1 year of relevant experience, and strong verbal and written communication skills.

Formación

• BA/BS in related human services field required.
• Minimum 1 year experience in developmental disabilities and/or regulatory compliance.
• Thorough knowledge of City, State and Federal regulations governing operation of relevant facilities and programs.
• Adheres to corporate compliance program and mandatory reporting; participates in compliance investigations.
• Excellent verbal and written communication skills.
• Ability to work flexible hours and/or days as necessary.

Responsabilidades

• Promote the mission, vision and values of CP unlimited.
• Develop, implement and monitor the program plan for each person receiving services.
• Chair and participate in annual, semi-annual and special team conferences.
• Complete required documentation and maintain accurate records for all residents.
• Attend all life plan meetings and implement program plans accordingly.
• Review data, provide feedback and train the team to ensure plans are followed; conduct site visits.
• Maintain systems, policies and protocols reflecting best practices.
• Participate in intake and screening of new residents.
• Review Life Plans to ensure supports are reflected.
• Distribute SAPs and complete PC Reports weekly.
