Job summary

Northeast Arc seeks a dedicated supervisor to provide supervision, opportunities, and skill development to participants in its community-based day and work support program. You will train and guide assigned program case managers, oversee a caseload, and ensure progress toward goals within a dynamic community setting.

You will evaluate progress, develop individual skills, maintain documentation, and coordinate transportation and volunteer placements.

Qualifications

• High School Diploma or equivalent required.
• Excellent organizational and interpersonal skills; effective communicator in writing and speech.
• Valid driver’s license and reliable transportation required.
• Must be able to lift up to 50 lbs and stand/walk up to 1 hour.

Responsibilities

• Evaluate individuals' progress periodically and adjust as needed to improve outcomes.
• Evaluate and determine participants’ interests and develop individual skills.
• Provide caseload oversight by attending meetings, maintaining contact, completing documentation, and ensuring progress.
• Create individual goals reflecting participants’ vision for skill development.
• Maintain required documentation (Daily Data, Monthly/Quarterly Progress Notes, Assessments, Confidential Records).
• Provide transportation to participants as needed.
• Assist in obtaining volunteer placements and coach participants in those placements.
• Attend and complete required training assigned by supervisor.

Job Summary:

Provides supervision, opportunities, and skill development to participants served in NeArc’s community-based day and work support program. Provides supervision, guidance, and training to assigned program case managers.

Major Responsibilities:

• Evaluate individuals progress periodically and make adjustments as needed to improve outcomes
• Evaluate and determine program participants’ interests and develop individual skills.
• Provides assigned caseload oversight by attending scheduled meetings, maintaining regular and professional contact with the participant’s team, completing all required documentation, and ensuring the participant’s progress in the program as determined by his or her goals.
• Creates individual goals to reflect the participant’s vision regarding skill development.
• Maintains all required documentation within required timeframes to include:

• Daily Data
• Monthly/Quarterly Progress Notes/6-month summaries in HCSIS
• Assessments
• Confidential Record updates

• Provides transportation to participants served in a personal or agency vehicle when one is available.
• Assists in obtaining appropriate volunteer placements for participants served and provides ongoing coaching support in these placements to ensure continued success.
• Attends and completes all required training assigned by the supervisor.

Qualifications:

• High School Diploma or equivalent required. 1 year of experience working with intellectually and/or developmentally disabled adults or other related experiences preferred.
• Excellent organizational and interpersonal skills. Effective communicator in both person and writing. Ability to communicate in English and or ASL via direct communication.
• Valid driver’s license and reliable transportation is required.
• Must be able to lift to 50 lbs. as needed.
• Must be able to stand/walk up to one hour uninterrupted.

Work Environment:

The environment consists of office space, program areas, volunteer sites, vehicles and various locations out in the community.
