Job summary

UAB Medicine is seeking a PRN to part-time Case Manager in Birmingham, AL. The role supports the interdisciplinary plan of care from admission to discharge, coordinating services across the continuum and addressing patient/family needs with a focus on smooth transitions.

You will collaborate with a care team, advocate for needed resources, and ensure accurate documentation and appropriate post-discharge options in a dynamic hospital setting.

Qualifications

• Licensure required: LBSW or LMSW by Alabama State Board of Social Work Examiners.
• Bachelor's degree in Social Work required.

Responsibilities

• Coordinates the interdisciplinary plan of care from admission to discharge.
• Creates care plans across the continuum incorporating patient/family preferences and values.
• Assesses and addresses legal, neglect, substance abuse, crisis and psychosocial needs.
• Advocates for resources and removal of barriers to care.
• Maintains dialogue with supervisor and care transition team for effective implementation and reevaluation.

Education
