The Health Care Authority for Baptist Health, An Affiliate of UAB Health System in Prattville, Alabama seeks an RN Case Manager (PRN) to coordinate care across discharge planning and utilization management. You will collaborate with physicians, nursing staff and the care team to optimize resources and patient outcomes.

Qualified candidates hold an active RN license with 2+ years of hospital patient care experience; a Case Manager certification is preferred.

Formación

• Degree in Nursing required, Bachelor's preferred.
• At least 2 years of hospital patient care experience.
• Active RN license required; Case Manager certification preferred.

Responsabilidades

• Coordinate care for assigned caseload with physicians and staff to optimize resources.
• Complete admission, continued stay, and discharge reviews per regulations and plan.
• Reconcile orders and monitor medical necessity for revenue protection.
• Collaborate with interdisciplinary team and payers to ensure continuity of care.
