UAB Medicine in Birmingham, AL seeks a licensed social worker to coordinate the interdisciplinary plan of care from admission to discharge. The role requires an LBSW or LMSW license and a Bachelor's degree in Social Work, with a Monday–Friday schedule and holiday/rotation moments at times.

Responsibilities include creating care plans, monitoring care via assessments, advocating for resources, and ensuring proper medical record documentation for post-discharge options.

Competenze

• Must be licensed LBSW or LMSW by the Alabama State Board of Social Work Examiners.
• Bachelor's degree in Social Work required.

• Coordinate the overall interdisciplinary plan of care for patient, from admission to discharge.
• Create plan for care across the continuum, integrating patient/family preferences and values.
• Monitor patient care through assessments, evaluations and/or patient records.
• Advocate for resources and removal of barriers.
• Maintain ongoing dialog with supervisor and other health providers to ensure effective implementation of health plan.
• Act as a resource for adequate medical record documentation, appropriateness of services as they relate to diagnoses, and treatment options for post-discharge care.
