Job summary

UAB Medicine is seeking an RN Case Manager to coordinate comprehensive care from admission to discharge at UAB St Vincent's Birmingham. The role requires active RN licensure in Alabama, a diploma in nursing, and BLS certification within 30 days of hire.

Qualifications

• Must be licensed as RN by the Alabama Board of Nursing.
• BLS certification obtained within 30 days of hire date.
• Diploma from an accredited school/college of nursing required.

Responsibilities

• Coordinate the interdisciplinary plan of care from admission to discharge.
• Create care plans across the continuum incorporating patient/family preferences.
• Monitor medical necessity and level of care via assessments and records.
• Advocate for resources and remove barriers to care.
• Maintain ongoing dialogue with supervisors and care transition team for reevaluation of health plan.
• Serve as a resource for medical record documentation and post-discharge care options.
