Job summary

Senior Community Care of Colorado PACE (Montrose) with Volunteers of America is seeking a Registered Nurse for a PRN, on-site role in Montrose, CO. The RN will partner with the Care Manager and Interdisciplinary Team to deliver comprehensive nursing care and support to PACE participants.

Responsibilities include initial assessments, ongoing care planning, coordination with the team, and on-call coverage as needed. Colorado RN license and CPR are required; BSN preferred.

Qualifications

• Graduate of a school of professional nursing.
• Current Colorado Registered Nurses (R.N.) License, CPR certification required, BSN preferred.
• Must have a valid driver’s license, proof of insurance and transportation.

Responsibilities

• Develops and implements a plan of care to meet participants’ needs and coordinates care between facility and interdisciplinary team.
• Conducts comprehensive initial nursing assessments for new PACE enrollees and periodic re-assessments.
• Provides general and skilled nursing care as assigned and documents patient status and responses to treatment.
• Participates in nursing on-call rotation for after-hours and weekend support.
