Job summary
Senior Community Care of Colorado PACE in Montrose, CO is seeking a Registered Nurse for PRN shifts. You will lead nursing assessments, plan and coordinate care with the Interdisciplinary Team, and deliver skilled nursing services in the community setting.
Requires CO RN license and CPR certification, BSN preferred. Must have valid driver's license and reliable transportation for community-based care. Join VOANS’ program dedicated to elder care and compassionate, purpose-driven service.
Qualifications
- Graduate of a school of professional nursing.
- Current Colorado RN license and CPR certification required.
- BSN preferred.
Responsibilities
- Develops and implements a plan of care to help participants remain living safely in the community.
- Conducts an initial nursing assessment and coordinates with the Interdisciplinary Team to create a care plan.
- Provides general and skilled nursing care as assigned and documents patient status and response to treatments.
- Participates in on-call nursing rotations for after-hours and weekend care.
