Qualifications

Responsibilities

• Develops and implements a plan of care to meet participants’ needs and coordinates care between facility and interdisciplinary team.
• Conducts initial nursing assessment for new PACE enrollees and re-assessments as needed.
• Performs general and skilled nursing care; follows physician orders and documents patient status.
• Participates in on-call nursing rotation for after-hours and weekend care.
