Job summary

Senior Community Care of Colorado PACE in Montrose, CO is seeking a compassionate Certified Nursing Assistant to support participants with daily living, restorative care, and home visits as part of VOANS family. This full-time role offers a stable schedule with varied days and occasional weekends.

The position pays $19.50–$23.50 per hour based on experience, along with comprehensive benefits and opportunities for career growth within VOANS’ PACE program.

Qualifications

• High School diploma or equivalent is required.
• Current Colorado CNA license is required.
• Minimum of one year experience with frail/elderly population required.
• Valid driver’s license, proof of insurance and transportation means required.

Responsibilities

• Provide and assist with activities of daily living, restorative and supportive care to PACE participants.
• Travel to participant homes as needed and collaborate with Interdisciplinary Team to monitor health status.

Pay Range: $19.50 - $23.50 per hour (based on previous work experience)

Schedules: Schedule: Full-Time|80 hours per pay period | Days and shift times will vary and includes occasional weekends and holiday rotation

Travel: Requires travel to participant homes. You must be at least 21 years of age!

What You Bring (Requirements)

• High School diploma or equivalent required.
• Current License as Certified Nursing Assistant (C.N.A) in the state of Colorado required.
• A minimum of one year's experience in working with the frail or elderly population required.
• Must have a valid driver’s license, proof of insurance and have means of transportation.

What You'll Be Responsible For

The Certified Nursing Assistant is responsible for providing and assisting participants with activities of daily living, restorative and supportive care necessary to the preservation of assisting participant’s in living independently in the community. Assists in monitoring and reporting the health status and providing a safe environment for the PACE participants of Senior CommUnity Care. Functions as a member of the Interdisciplinary Team. This position may require travel to participant homes.

• Assists participants with Independent Activities of Daily Living (IADL's) including but not limited to grocery shopping, cleaning, changing bed linens, laundry, meal preparation and other routine household care as assigned. Reports changes in participant's condition and needs to the Personal Care Supervisor (or designee).
• Assists with participant activities in the Day Center, including meal service and clean-up.
