Job summary

Senior Community Care of Colorado PACE (Montrose) is seeking a Home Care Manager in Montrose, CO. This role oversees day-to-day Home Care operations, collaborates with the IDT, and ensures high-quality, compliant care for elderly participants.

A Bachelor’s Degree or equivalent, CNA, CPR, and state certifications are required, with management experience preferred. Salary range is $63k-$92k annually, with a Monday-Friday 8:00am-5:00pm schedule and occasional weekends.

Qualifications

• Bachelor’s Degree or equivalent education and experience.
• Minimum requirement of C.N.A. certification.
• Current State Specific certification as a Licensed Practical Nurse, Registered Nurse, Occupational Therapist, Physical Therapist or other licensed professional preferred.
• CPR certification required.
• A minimum of one year’s experience in working with the frail or elderly population required.
• Minimum of two (2) year’s management experience in a hospital, nursing home or community-based setting.
• Home health experience preferred.

Responsibilities

• Oversees daily operations of the Home Care department to ensure high quality, safe care.
• Participates in Interdisciplinary Team meetings and represents Home Care.
• Oversees scheduling and performance of home care staff and coordinators.
• Coordinates plan of care with IDT and ensures adherence to approved plans.

Education

Bachelor’s Degree or equivalent education and experience

CPR certification

Driver’s license

1 year working with frail or elderly population

2 years management experience in hospital/nursing home/community-based setting
