Job summary

Medical City Fort Worth RN Case Manager is a PRN role providing clinical expertise and coordinating plan of care, length of stay, and safe discharge planning in collaboration with the care team. You will assess psychosocial and medical needs, develop strategies to mitigate readmission risk, and ensure a smooth transition of care.

The position requires TX RN licensure, associate degree or diploma, and 2–3 years of hospital or case management experience.

Qualifications

• RN license in TX
• Associate Degree in Nursing or Nursing Diploma required; Bachelor's preferred
• 3+ years hospital nursing or 2+ years case management experience
• Case management certification preferred
• InterQual experience preferred

Responsibilities

• Perform comprehensive psychosocial, medical, and discharge assessments
• Coordinate care plan and discharge planning with multidisciplinary team
• Facilitate patient throughput and safe transition of care
• Refer to Social Services when appropriate
• Advocate for patient rights and ensure culturally/ethnically appropriate care
