Job summary

HCA Healthcare is seeking a skilled RN Case Manager for Medical City Arlington. This full-time day shift role focuses on coordinating care, assessing patient needs, and ensuring safe, efficient discharges.

You will collaborate with a multidisciplinary team to promote high-quality outcomes, manage length of stay, and engage with Social Services as needed. Experience with InterQual is preferred, and Texas RN licensure is required.

Qualifications

• Registered Nurse with current TX state license required.
• Associate Degree in Nursing or Nursing diploma required, Bachelor's Degree in Nursing preferred.
• 3+ years of clinical hospital nursing experience required OR 2+ years experience in case management.
• Certification in case management preferred.
• InterQual experience preferred.

Responsibilities

• Performs comprehensive assessment of psychosocial, medical and discharge needs.
• Reassesses patient’s clinical condition and readmission risk; educates on resources.
• Coordinates plan of care and drives discharge planning with multidisciplinary team.
• Collaborates with Social Services for post-acute medical needs and level of care.
• Refers to Social Services when psychosocial risk factors identified.
• Involves patient/family in identifying needs and goals.
• Evaluates progression using InterQual criteria and escalates issues as needed.
• Makes referrals to third party payer and disease/case management programs.
• Facilitates patient throughput and effective care transitions.
• Documents recommendations, discharge plan, and coordination activities for the team.
• Aligns patient needs with available resources for safe discharge.
