Job summary

Encompass Health Corporation is seeking a Case Manager to coordinate patient care from admission to post-discharge, ensuring high-quality rehabilitation outcomes. You will work with an interdisciplinary team to manage resources, plan care, and support timely discharges for patients in our Fort Worth, TX facility.

The role emphasizes assessment accuracy, CCM/ACM certification, and active state licensure. Experience in rehabilitation and a BSN (or higher) is preferred to excel in this position.

Qualifications

• Must be qualified to independently complete an assessment within the scope of practice.
• Active license in the state; CCM® or ACM™ certification required or to be obtained within two years.
• Bachelor's degree required for all other eligible licensed or certified health care professionals; graduate degree preferred.

Responsibilities

• Coordinate patient care from admission to post-discharge with interdisciplinary teams.
• Oversee plan-of-care decisions and discharge planning in collaboration with care teams.
• Participate in utilization review processes and documentation within the patient record.
• Assist in developing discharge goals and contingency plans to avoid delays.
